At MedCentric Financial Network (MFN), your privacy is our priority.

We recognize that you expect your personal information to be handled in a professional, confidential manner. We have adopted the following policies to safeguard your privacy and to explain the circumstances under which we may collect, maintain and use any non-public personally identifiable information that you may provide us.

We collect personal information to:
1) provide you with services, including requested referrals to advisors;
2) maintain quality control;
3) fulfill contractual obligations between MFN and its advisors;
4) meet our regulatory obligations.

The type of information we collect may include:

•For those visitors to our website who choose not to provide personal information, we only collect such basic data as your IP (Internet Protocol) address, operating system, and the time and date of your visit. We may also collect some data about your visit through the use of "cookies". This information is personally identifiable only if you chose to identify yourself on the site.

•If you use this website to obtain a referral to a MFN affiliated advisor, we will ask you to provide personal information, including your name, address, work phone number, your employer affiliation, and your acknowledgement of:

1) a fee sharing disclosure document and,

2) the advisor's Form ADV Part II.

This data is used to provide services to you, to monitor the referral process to our advisors, and to fulfill our regulatory obligations under the Investment Advisor's Act of 1940.

•If you engage an advisor to whom MFN referred you for financial planning services, such as the Personal Financial Review or a comprehensive financial plan, then your personal information will be used by MFN and the advisor to facilitate making these services available to you. If you are part of an employer sponsored financial planning benefit program, then your data may be used to help process any payment made on your behalf by your employer, or for quality control purposes.

•If after being referred by MFN to one of our affiliated advisors, you engage that advisor for asset management services through Charles Schwab, SEI Investments, or other custodians, MFN may periodically reference your investment accounts at each custodian. This information is used only for the purpose of fulfilling the contractual obligations between MFN and your advisor, and for quality control purposes, or as outlined below.

We take steps to protect the security and confidentiality of the personal information we collect. MFN does not share non-public personal information of any kind about referrals or clients with unaffiliated third parties with whom MFN has no contractual business relationship for their independent use unless (1) the client gives MFN written permission, (2) it is necessary to protect against fraud, comply with a subpoena or other court order or is otherwise required by law. We do not sell or rent information about referrals or clients to outside parties for any purpose.

Further, we restrict access to the personal information of referrals and clients to those employees who need to know that information to provide products or services to you, and maintain strict physical, electronic, and procedural safeguards to guard your non-public personal information.

If a person becomes an inactive customer, MFN will continue to adhere to the privacy policies and practices as described in this notice.
